POSITION: Medical
Case Manager
Medical Case Manager - Part time positions
POSITION PURPOSE: To provide a
range of client-centered services that link clients with health care,
psychosocial, and other services. Provides the coordination and
follow-up of medical treatments and ensures timely and coordinated
access to medically appropriate levels of health and support services.
Coordinates the care delivery of a selected population of persons with
HIV consistent with the needs and wishes of the client and within the
IAP parameters, policies and procedures and applicable State and Federal
regulatory provisions.
RESPONSIBILITIES: Under the
direction of the Director of Case Management the Medical Case Manager
will:
§
Act as a liaison between the patient and health care
providers.
§
Complete a face-to-face intake assessment on each new
patient with HIV.
§
Develop a written service plan specific for each client
which shall include but not be limited to:
a. medical care needs
b.
nursing care needs
c.
psycho-social needs
d.
financial needs
e.
public/private benefit assistance
f.
transportation
g.
nutrition
h.
education needs
i.
family support system
§
Make referrals for appropriate care and assures that
needed care is being delivered.
§
Update and modify the plan of care on an ongoing basis;
monitoring the patient's progress pro-actively.
§
Coordinate and facilitate interdisciplinary case
conferencing for the provision of care to persons with HIV.
§
Communicate on an ongoing basis with client, family and/or
significant others consistent with identified need and case management
guideline.
§
Assure that accurate client records are being maintained
and reflect medical, nursing, health education and psychosocial
interactions with the patient.
§
Assist the client, family and other health care providers
in problem solving.
§
Teach and/or coordinates educational sessions.
§
Evaluate eligibility for
funded services as per contract requirements.
§
Authorize needed services
and payment for those services as per contact expectations.

Complete necessary enrollment, disenrollment and
authorization procedures for funded care programs as per contract
expectations.

Other duties as assigned.
QUALIFICATIONS:
-
Knowledge of Public Health principles, nursing
process application, psychosocial, medical, nursing, nutritional and
education needs relating to HIV infected individuals.

Knowledge of principles of leadership and
supervision.
-
Ability to relate to people with HIV in a
non-judgmental supportive manner, perform an intake interview, and
utilize the nursing process.

Ability to access and work in conjunction with
health care agencies and systems and establish a leadership role and
collaborative working relationships.

Have sensitivity to HIV related issues.

POSITION: Mental
Health Therapist Part time
POSITION PURPOSE: To provide
short term mental health services to clients affected by HIV/AIDS.
RESPONSIBILITIES: Under the
direct supervision of the Director of Behavioral Health Services the
Mental Health Therapist will:
§
Screen prospective clients to determine appropriateness for
program services; provide orientation sessions for clients deemed
appropriate.
§
Conduct intake assessments; prepare diagnostic evaluations
which include obtaining personal, social, emotional, and substance abuse
history; determining the cause, nature and severity of the client’s
current mental status.
§
Develop, evaluate and implement goal-oriented treatment
plans.
§
Provide direct treatment services such as individual and
group counseling to a difficult and complex client population.
§
Provide clinical supervision of BA/Masters level interns
as assigned.
§
Intervene in crisis situations when a client exhibits
acute psychiatric symptoms which present an immediate threat to the
client.
§
Develop and/or implement minor and major program changes
related to such areas as group structures, treatment goals and
directions, the incorporation of newly mandated services, and contract
services.
§
Provide consultation to other staff members on a client’s
history, mental status and previous treatment, and on the community
resources available for the treatment of a client.
§
Participate in the staff review of a case; recommending
changes in therapeutic techniques, environment, and/or charting to
facilitate client recovery.
§
Recommend when to terminate professional counseling
treatment; develop follow-up plan and/or revised post treatment
techniques and activities.
QUALIFICATIONS:
-
Masters Degree and associated license in counseling
or social work preferred.
-
Experience with counseling individuals with HIV
helpful.
-
Knowledge of counseling techniques, DSM-IV diagnoses
treatment planning and documentation of services.
-
Ability to relate with a wide variety of
individuals; problem solve.
